There are many medical anti-aging options available today, but most are less effective and more time consuming than the newest batch of non-invasive cosmetic medicine. With the popularity of these treatments skyrocketing, it may not be difficult to find a service provider. Before booking an appointment at your local medical spa, skin clinic or laser treatment center, remember, this is a medical procedure. Taking the time to locate a qualified practitioner within a medical spa ensures your safety and satisfaction.

Use the following tips to select your provider:

1. Opt for a consultation before booking any treatment. Use this opportunity to check out the medical spa, skin clinic or laser clinc physicians and staff.

2. Pay attention to your comfort level with the staff and level of customer service provided. Are your questions and concerns addressed to your satisfaction? Are you treated warmly and with respect? It is important that you feel comfortable and trust your provider.

3. Inquire if a physician is always present in the facility and sees every patient before they are treated. State laws (as well as medical ethics) require that a medical doctor physically be on site to monitor laser or medical treatments. If there is not a physician on-site, they are breaking the law and putting their patients at risk.

4. Determine the specialty of the staff physicians. Physicians who specializes only in non-surgical / non-invasive medicine, is preferable to a general dermatologist or plastic surgeon, for example. Many doctors are buying a piece of technology, taking an hour long class, and branding themselves an expert. Look around. If the clinic is run by a plastic surgeon, how can he specialize in non-surgical treatments?

5. Ask how many different medical devices they have. If the answer is just one or two, they aren’t really specializing in non-invasive medicine and are probably an “add-on” clinic. Look for a clinic that has multiple devices of different types.

6. Make sure that your skin type is accurately determined if you’re having a laser or IPL treatment. If your skin type is determined with a questionnaire, go elsewhere. Questionnaires have been determined to be inaccurate 70% of the time. The best clinics use a special “skin-type” meter (spectrophotometer) to scientifically determine exactly how much melanin is in the treatment area.

7. Discuss any issues for your skin type that may arise during treatment. You’ll want to know side effects to expect, or if a specific piece of equipment is best suited for you. A knowledgeable professional is willing to discuss your concerns, and is familiar with options.

The key to finding the right provider is to actively research your options, and voice your needs. You want to only work with a specialized medical practice; one that addresses your concerns and welcomes your questions. Managing your treatment will improve the level of care you receive, and your satisfaction with the procedure’s results.

With new products entering the market like Dysport and Evolence, and other exiting like ArteFill, it is important that your staff be knowledgeable of the products used in injectable procedures.

Although this chapter from the Advanced Medical Aesthetics Training Manual wasn’t actually requested in the training manual I wrote for the academy students, I felt it was extremely important for the entire medical spa staff, especially for medical estheticians, to know the difference amongst the various treatments used to alleviate the appearance of wrinkles and folds, and the difference between dynamic and static lines.

Medical estheticians have the most “face time” with clients, examining it with magnifying lenses, treating it with exfolliants, moisturizers, galvanic current, high frequency, various acids, etc. They understand the realm of fine lines and wrinkles and know what can be alleviated with topical treatments such as facials, peels, microdermabrasion and laser resurfacing, and what cannot.

I cannot tell you how many patients call or come into our office with no knowledge of the difference between paralyzing agents such as Botox and Dysport, and dermal fillers. We take it for granted that we know the difference, but injectable products can be very confusing to the lay person. Because the aesthetician works with the client’s skin, they should be well versed in the differences in the two categories of injectables and the products that lie within. Estheticians become your most powerful education source for your clients which ultimately leads to an upsell for them (with possible commission).

Many people think all fillers products are the same, nor do they care to understand the difference between hyaluronic acid or calcium hydroxylapetite. All they truly care about is “how long will it last” and will the “results look fake”. Only with proper education can the medical esthetician effectively and intelligently discuss the differences in the filler products, educate the patient about the effects and lasting results, and help lead the patient to the right product, or combination of products, for their expected outcome.

This information in this chapter is very important for your medical spa front desk/phone people as well!

Laser Hair Removal FAQ from skin clinics, plastic surgery clinics and dermatology centers.

All laser treatments technically operate the same way when performing a hair removal treatment. The Intense Pulsed Light Device (IPL) or cosmetic laser uses light energy to destroy the hair folicle.

It is important that patients about to undergo a laser hair removal treatment or procedure not to wear dark clothing near the area to be treated. While very unlikely, the pigment in the clothing could potentially be absorbed by the Intense Pulsed Light Device (IPL) or cosmetic laser light and can result in a burn. Patients should be instructed to refrain from using any body washes, lotions, cosmetics, or any skin medications the day of the procedure.

Some patients may prefer to have a topical anesthetic applied prior to the procedure. If any residue of the anesthetic remains, it should be wiped clean from the skin. Often a cooling gel or other cooling apparatus is applied to the skin.

Patients with darker skin types or suntanned skin could experience a paradoxal response where the laser or light therapy may stimulate more hair to grow instead of causing follicular destruction. Patients with dark or ethnic skin types should be spot tested a month before proceeding with a full treatment. A patch test should be performed on the intended treatment area. After waiting 10 to 15 minutes, the area is checked for unusual redness swelling, bruising, or any other undesirable responses. If the area emits a slight reddish hue with no other abnormalities, the device settings are optimal.

Some Intense Pulsed Light Devices (IPL) or cosmetic lasers, IPL in particular, require a second pass over the intended treatment area at a different parameter. Before proceeding with the second pass, the skin should be carefully examined for adverse effects.

Laser Hair Removal FAQ: Post Treatment

After a aesthetic laser or IPL hair removal procedure the patient may experience a mild burning sensation and redness. Exposure to ultraviolet light (sun, tanning beds/booths) should be avoided. A sunscreen with an SPF 20 UVA and UVB protection should be applied to the skin prior to leaving the office.

In the case of burning or blistering a topical antibiotic medication should be applied and the laser clinics physician should be consulted. Unfortunately, some patients may experience temporary pigment changes such as lightening or darkening of the skin.

After the Intense Pulsed Light Device (IPL) or cosmetic laser hair removal treatment, the portion of the hair which remains under the skin will be expelled. The patient should be made aware that an actively growing hair is the only follicle which will respond to laser and light therapy. This is especially more noticeable in areas of the body which has thicker or more coarse hair such as the axillae and genetalia. It is important to educate the patient about the process of hair follicle destruction.

Laser hair removal patients should also be informed that no laser or IPL device can destroy all of the hair follicles… especially after only one treatment. Some hairs are in their dormant state and can’t be treated, some have been placed into a prolonged dormant state due to the procedure effects, and others have been transformed into vellus hairs (light, fine hairs). Several treatments are required at monthly intervals for an optimal outcome. A maintenance procedure performed at yearly intervals is recommended for permanent hair removal.

What is it?

Before advanced laser hair removal devices entered the market, electrolysis was the only form of long term hair removal. Electrolysis is still performed today even though it is painful to the patient and invasive to the skin, allowing for the possibility of infection. During the electrolysis procedure, a fine needle is inserted into the hair follicle, delivering a galvanic current to destruct the stem cells in the follicle. Unfortunately, this painstaking process is performed one hair follicle at a time. Maintenance procedures are often needed for many years after the initial treatment. In contrast, with laser and intense pulsed light (IPL) technology, hundreds of hair follicles are treated at a time and vary upon the spot size (the size of the tip of the device delivering energy). These laser hair removal procedures are non-invasive and take only minutes to perform instead of the many hours with electrolysis.

What is Melanin and what part does it play in advanced laser hair removal?

The most important consideration prior to hair removal a procedure is the patient’s skin type and hair color. Laser hair removal is based on selective photothermolysis (using light and heat for destruction of a hair follicle) with melanin being the target chromophore (chemical grouping that produces color). The difference between the absorption of laser or light energy by melanin in the hair follicle and melanin in the epidermis determines the amount of energy that can be safely used without harming the skin. Light skinned patients with dark hair are considered to be the ideal patients for hair removal On the other hand, a patient with very dark skin and gray or white hair, would have little if any response. Since melanin in the hair shaft and follicle are the target source for laser and IPL hair removal procedures, it is important for patients during their initial consultation not to tweeze, wax or use any depilatory device prior to hair removal. The patient can, however, shave the area to be treated (which preserves the chromophore in the follicle), or can use hair removal creams. Hair which is protruding from the skin can singe during a laser or light treatment which causes an unpleasant smell and possible flares.

Advanced Laser Hair Removal: What to expect.

Most devices technically operate the same way when performing an advanced laser hair removal procedure. The parameters on the equipment should be adjusted according to the history and skin analysis of the patient. The patient should be instructed to refrain from using any body washes, lotions, cosmetics, or any skin medications the day of treatment. Some patients may prefer to have a topical anesthetic applied prior to the procedure. If any residue of the anesthetic remains, it should be wiped clean from the skin. It is also important for patients to avoid dark clothing near the area to be treated as the dye in the clothing can be absorbed by the laser or light resulting in a painful burn. A patch test should be performed on the intended treatment area. Several monthly treatments are recommended for the best results and a yearly maintenance procedure is recommended.
